It is a fairly straightforward phrase. When you say someone has put on weight you basically means you have gained weight, have become fat. It is a common phrase, more so in conversational English. Let’s look at some examples:

Thus, put on weight simply means become fat. You can use it in formal as well as informal settings – you can say it to your boss as well as amongst your college friends.
